To apply for service, click on Application for Service button, below. You will be taken to a new page where you can complete the form and submit it online.
You must be sure to sign the 3rd page of the Application for Service. When filling out the form you can look to the left side of your screen and see the 3 pages you will be completing
Once you have submitted the form, wait 15 minutes and then contact the office at 979-589-3030 EXT. 7 to pay the necessary fees over the phone with Mastercard, Visa, Discover, or American Express.
Fees must be paid in full before service can be activated. If you wish to print the forms go to the links at the bottom of this page. If you don't find the form or report you are looking for, please give us a call or stop by and visit us. We will gladly provide you with the information you need.
